Directions to pay with a check or money order

First go to the order form page.  Print the form out with your printer.  Then browse though the store and put your choices on the order form.  Fill the form out completely.  Make sure you have your address on the form.  Mail the order form along with check or money order to the address on the form.  Or call 859-358-0150 to place your order by telephone, then mail your check or money order to:
